It focuses on helping people move through complex healthcare, housing, and support systems in a way that promotes stability, safety, and long-term well-being.
Support During Life Transitions
This work is especially helpful during moments of transition, including:
Hospital or facility discharge
Changes in health or functional needs
Housing instability or unsafe living situations
Caregiver burnout or lack of family support
Situations where multiple systems must be navigated at once
During these times, navigation support helps individuals and families understand options, prioritize next steps, and avoid unnecessary delays or gaps in care.
What Health & Housing Navigation Includes
Support is tailored to each individual’s situation and may include:
Coordinating care across healthcare providers and service systems
Navigating Medi-Cal, public benefits, and community-based resources
Identifying appropriate housing options, including affordable and Medi-Cal–friendly alternatives
Supporting transitions into assisted living, independent living, or other safe housing
Providing follow-up coordination to support ongoing stability
