We partner with case managers, discharge planners, social workers, and community-based organizations to support timely, safe transitions for older adults and vulnerable individuals. Our services help address common barriers related to housing availability, affordability, care coordination, and post-discharge support.

We assist with complex placement cases, hospital discharge planning, housing navigation, and community health coordination. Our role is to complement existing care teams by helping move cases forward, reducing delays, and supporting continuity of care.

What Health & Housing Navigation Includes

Support is tailored to each individual’s situation and may include:

  • Coordinating care across healthcare providers and service systems

  • Navigating Medi-Cal, public benefits, and community-based resources

  • Identifying appropriate housing options, including affordable and Medi-Cal–friendly alternatives

  • Supporting transitions into assisted living, independent living, or other safe housing

  • Providing follow-up coordination to support ongoing stability

The goal is not just to identify resources, but to help people successfully move through the process.

Community health work in partnership with la care

Harlow Senior Resources provides Community Health Worker (CHW) services to help eligible individuals navigate health and social needs that may impact their overall well-being. We work alongside healthcare providers, case managers, social workers, and community organizations to connect individuals with resources and support that can help address barriers to care.

CHW support may include:

  • Transportation and benefits resources

  • Support following hospital or skilled nursing discharge

  • Coordination with caregivers and care teams

  • Connection to appropriate community-based services

  • Health system and care navigation

  • Housing and senior living resource navigation

  • Food and community resource connections

  • Durable medical equipment (DME) resource navigation

How Our Process Works:

We start with a conversation to understand the situation, identify priorities, and coordinate next steps across care, housing, and community resources.
